Fluoroscopically-guided Intra-articular Cortisone Effective in Frozen Shoulder

According to a study published in the 'Knee Surgery, Sports Traumatology, Arthroscopy', a fluoroscopic-guided intra-articular injection series of cortisone is an effective treatment option in frozen shoulder syndrome for fast pain reduction and increased range motion.

Frozen shoulder or adhesive capsulitis is a chronic debilitating painful condition in middle-aged which limits the mobility of the shoulder joint. The capsule of the shoulder joint become inflamed, stiff and leads to formation of adhesions.

Even thought he exact cause is not known, predisposing factors include diabetes, Injury, breast surgery, lung cancer etc.

Capstone Commentary on ASTRO and when the freeze thaws ...

ASTRO, the radiological oncology conference was bigger this year than in the past. look for my discussions in radiology, radiology technician, mammography, PET, MRI and other groups on ASTRO. i spoke to a group of RN consultants who were out of work. RNs out of work? heh? we're like 800,000 short of nurses, how can this be? hospitals are cutting staff for fear of heatlhcare reform, medicare reimbursement cuts, etc. saw the same at ASHP (pharmacist meeting). pharm Ds are getting cut from hospital pharmacies, another huh? what the? pharmacists reduce medical errors, save cuts, and increase revenue, why then? c-suite hosp admins are telling directors to cut, and instead of themselves, they cut staff. crazy. vendors are telling me again, same as last several meetings they have budget freezes, not seeing customer activity, so when does the freeze thaw?

Your Gut Knows Best

DotMed reports research that is becoming familiar: doctors know instantly the status of a patient without engaging in long investigations. Experienced ER doctors faced with a patient with chest pains often have a "gut instinct" about the patient's condition that is often more accurate than detailed investigations.

However, try telling that to an insurance company or a litigation lawyer wanting to know why the patient was not subject to long expensive tests.

So the doctor may know, but most doctors would prefer to call up lots of tests so that they are seen to do their job.

While the research applies to ER, there is a wider impact on imaging. As patients read more about MRI or CT, they expect the doctor to test them, using up time and expensive resource. The costs of healthcare are driven as much by patient expectation as by the need for doctors to avoid later malpractice suits. Ipso facto, patients get scanned even when the doctor knows it is unnecessary.

In time, the danger is the doctors lose that gut feeling and simply rely on technology to give them the answer. Monkeys anyone?
