Patient Centered Medical Home Question

Here is a question sent in by a Placebian:


Doug,

The Sept-Oct 2010 issue of Family Practice Management (FPM) had an article about the Patient-Centered Medical Home. I have read about this and heard about it before. However, after reading the article in FPM I am even more confused. This is an actual paragraph from page #33:

“Primary care transformation is more about learning how to become a learning organization that creates an emergent future than it is about learning from experts on how to build something already known. The level of change needed is daunting and requires tremendous motivation of all practice participants, defining new roles, understanding the local landscape, and paying attention to multiple relationships. Future PCMH recognition and certification processes should focus more on patient-centered attributes and the proven, valuable key features of primary care than on the features of disease management and information technology. The PCMH represents the essentials for better primary care, the improved delivery of chronic care, and active partnership with informed patients synergized by appropriate use of information and communication technology."

What a bunch of gibberish. That sounds like something a hospital administrator would try to pass off as meaningful dialog..uh, I mean factual information (it is starting to rub off on me!). What does any of that mean? Can you explain exactly what a Patient-Centered Medical Home is? I know a lot of money is being thrown around to help us become one. What do you have to do to become a Patient-Centered Medical Home? What does it mean to the doctor and the patient?

Anyway, I thought you might be be able to use this stuff in a future Placebo Journal.

Editor's Response: Greg, the language they are using in the article is called ADMINISTRALIAN. I think the paragraph you quoted is a perfect illustration of what a PCMH is. The best way I can explain it is to take an expression from the show Seinfeld. Remember how it was a "show about nothing"? Well, the Patient Centered Medical Home is a concept about nothing. Think of it as an enigma, wrapped in a riddle and then dressed in a vest.
